Information collected

Review submissions contain a course identifier, rating, recommendation, written feedback, role, chosen display name, email and publication declaration. The form and hosting/security systems may record IP address, browser details and timestamps. We do not ask for patient data or attendance uploads in the public form.

Use and visibility

We use this information to moderate reviews, prevent abuse, follow up and publish feedback according to your display choice. Email addresses are not displayed publicly. Administrators can access original submissions. Reviews are stored both in Fluent Forms and the moderation records.

Your choices

Contact info@dcr.com (placeholder — not monitored) to request access, correction or removal. We may verify the request. Requests must be processed in both Fluent Forms and the review records, with backup retention considered.

Cookies and service providers

The intended launch experience uses essential functions without advertising or analytics. Hosting, security, form and email processors must be documented, and any optional tracking audited and consent-controlled before activation.

Retention

Proposed policy for approval: review rejected and unverified submissions after 90 days; retain approved reviews while relevant; remove attendance evidence promptly after verification; review inactive records and backups on an agreed schedule. These periods are not yet configured as automatic deletion.
